We are pleased to report that Sussex Community NHS Foundation Trust‘s new community diabetes service, named ‘Diabetes Care for You’, is now available to people aged 18+ who live in Brighton & Hove and the High Weald, Lewes and Havens area of East Sussex and that Crowborough hospital has been selected as one of the sites for delivery.

This new consultant-led service is available for adults living with Type 1 and Type 2 diabetes and helps people to live well by offering advice and support to manage diabetes and meet personalised goals.
